How to write a birth plan?

This is my first Pregnancy so I’m new to the whole writing of a birth plan. I’ll be 36 weeks on Tuesday and I want to have my birth plan written before I’m 37 weeks. I Already know what I want to happen and stuff, but I don’t know if I need to talk to my OB to write that stuff out on a special paper so it gets put in my chart, or if I just write it down and put it in my hospital bag. So how do I go about writing it? Thank You!

Huggies.com has some really good ones. That’s where I made mine. It asks you what you want and don’t want to happen then just press print. Babycenter.com has one, but I liked huggies one better.
